A Clinical Observation of 9 Cases of Epidural Hematoma without Skull Fracture

Abstract
A group of 9 cases epidural hematoma were reviewed with partibular attention to the epidural hematoma without skull fracture.
All the cases showed no evidence of skull fracture on plain skull X-ray films or on operations. In these cases there were difficulties to make early diagnosis with plain skull films but confirmed only with cerebral angiographies. The prompt angiography is simple and more valuable procedure, than trephination to rule out other combined complications in the doubtful cases.
The majority of type of injuries causing epidural hematoma without skull fracture were falling and traffic accident. Six of 9 cases showed definite lucid interval lasting from I hour to 20 hours, whereas the lucid interval predominated in 1-2 hour group.
In 3 of out cases the responsible vessel to cause the epidural hematoma were middle meningeal artery and its branches. One was caused by dural sinus tearing, however, 5 of them was unknown.
